Welcome to the Transition Resources for Youth website, or simply, tr4y.

This website offers youth, families, educators and agencies high quality transition tools and resources designed to improve and enhance the lives of youth with disabilities. We are presently working to complete the tools and resources on this website. Please note there may be changes and updates to the tools and information pieces on this site. TR4Y User Groups
All user groups will find both “Tools” and “Resources” on the tr4y website that relate specifically to their area of interest. The tr4y website is designed for a variety of user groups. Click on the links below to learn more about what TR4Y can offer to enhance your knowledge and skills about transition planning and services.
Getting Started
Using TR4Y will assist users in enhancing the transition planning process for improved post school outcomes of youth with disabilities. As you get started using the website, determine how you will use the website and if you need to register for a TR4Y account.
Search Resources
There are currently over 600 resources in the TR4Y repository, with new resources being added weekly. There are two ways those using ww.tr4y.org and use the resources. The TR4Y website has features that are available for the public to search and view both without first creating a TR4Y account. If a user would like to print and save located resources, a TR4Y account must be created first.
